CHW Provider Toolkit

Community Health Workers in Florida Community Health Workers (CHWs) serve as a link between Florida’s communities and health care delivery systems, facilitating access to services and improving the quality and cultural competence of service delivery. CHWs are uniquely qualified to achieve these goals even among groups that have traditionally lacked access to adequate care because they share the same community, personal, cultural, linguistic, and other characteristics of those they serve.
