Health Education in Transitional Care Management and Care Coordination
Join us for an engaging session on focused on the vital role of health education in Transitional Care Management (TCM) and care coordination. This course equips participants with practical, patient-centered strategies to improve outcomes and reduce readmissions. It also highlights the collaborative role of Community Health Workers (CHWs) and other healthcare professionals in delivering effective team-based care.
Participants will earn 1.5 Continuing Education (CE) units upon completion.
