Welcome to the Community Health Integration (CHI) Referral (CHI) System
This referral system connects individuals and families of all ages, abilities, incomes, and lived experiences to community-based programs and support services that help prevent and manage chronic disease, improve access to care, and promote healthier outcomes statewide.
Our web-based platform provides CHW coordinated, person-centered connections to trusted community resources. Guided by Community Health Workers (CHWs)—frontline professionals who bridge communities and health systems—we facilitate referrals that expand access to primary and preventive care through Federally Qualified Health Centers (FQHCs), charitable clinics, and rural health providers, particularly for individuals who may be uninsured or underinsured.
Through this CHW-centered approach, individuals are connected to the right community-based services at the right time—strengthening care coordination and improving overall well-being.
