Accurate, Effective, and Efficient Completion of Social Needs
This training session will equip participants with the skills to accurately and efficiently complete social needs referral forms and effectively use platforms like Unite Us, and others to track patient services and reporting. The session will also highlight how to connect patients with community resources and integrate social services into care coordination. Participants will earn 1.5 CE units.
