Facilitating Social and Emotional Support in TCM and Care Coordination
Participants will explore skills that help to build trust, support patient well-being, and address the social and emotional needs that impact health outcomes. Emphasis is placed on the role of the CHWs and interdisciplinary care teams in delivering compassionate, culturally responsive, and coordinated support.
